Is it safe to get the Covid vaccine if I've had filler?

Many friends and patients have asked me this question in recent days. The short answer: based on what we know at the moment, Yes and Yes. In fact, the American Society of Dermatologic Surgery put out guidance on Dec 29, 2020 that “patients already treated with dermal fillers should not  be precluded from receiving vaccines, and those who have had vaccines should not be precluded from receiving dermal fillers in the future.” 

This concern is based on reports of three people who had facial swelling within 1-2 days of getting the Moderna Covid vaccine. Two had cheek filler up to 6 months before; the third had lip filler two days after the vaccine, then developed lip swelling. All cases resolved with cortisone. Nome of these were anaphylaxis (a serious, potentially life-threatening allergic reaction).

What does all this mean?

  • We know that some fillers can cause delayed facial swelling. While uncommon, I have seen it three times in 26 years of injecting fillers.  In my experience, the risk is higher with certain brands such as Voluma, which is often used in cheeks and which I stopped using in my patients three years ago for this very reason.  Therefore I hope Moderna discloses which fillers were used in these cases.  

  • Infections and other vaccines such as the flu shot have been associated with facial swelling in those with filler. As of this post, more than 4 million Americans have had the Covid vaccine. Given these numbers, tbe risk of facial swelling after Covid vaccine is less than the reported risk of swelling after flu shot (1 in 1.3 million).

  • As with all decisions regarding fillers, please discuss any concerns with a medical doctor who can evaluate your medical history and ensure you’re receiving the safest treatment possible. Plus, a physician is best equipped to handle any potential reaction or side effects.
